Communications Graphic Design Project Request Form

When you have a need for graphic design services, use this form as a checklist and as a way of ensuring your project is properly entered into the production queue.

Project checklist – Do you have these ready?

List the size and format of the graphic project, if more than one size is needed, such as for a social post, include all.

Who is the target audience for this piece? Students? Donors? Scientists? The community?

Purpose – What is the goal for this project? i.e., Promote an event, drive web traffic, recognize someone.

Message – Provide the text or request assistance developing your message.

What’s the call to action? Do you want people to register, to donate, to comment, to read more? Include link. If a new webpage is needed, create an additional entry under the web form.

Do you have visual assets? Photos, video, related links? Please provide all.

Reviews and approvals. Be clear about who should sign off on final graphics.

Deadline? Delivery can take three to six weeks, depending on the projects ahead of yours.

Liaasing with UF Health Creative Services for various media and design projects.
Photography and videography needs.
Mention on our social media channels.
All media inquiries.
Marketing needs for symposia, events open to the public
Updates to the scripps.ufl.edu website.
External communications needs such as speaker’s requests.
